Did you consume or use a product made by ByHeart?

Keep the package, lot code, receipt, order confirmation, or a photo if any of those records are available.

Did a doctor or lab confirm Infant Botulism?

Medical records and lab results help document the illness, but an attorney can still review the facts if testing was not done.

Did you receive medical treatment?

Hospital visits, ER visits, or doctor appointments count.

Do you still have purchase, product, or exposure records?

Choose the closest answer. Missing a receipt does not automatically prevent an attorney from reviewing the claim.

Infant botulism occurs when Clostridium botulinum spores colonize an infant's immature gut and produce botulinum toxin. ByHeart Whole Nutrition Infant Formula was contaminated with C. botulinum Type A spores traced to organic whole milk powder in the formula. This is the first-ever outbreak of infant botulism linked to powdered infant formula. All 48 affected infants required hospitalization and treatment with BabyBIG antitoxin.
